Patrick Forma, 73, passed away on Monday, May 25, 2020.

Currently a resident of Greenville, SC, Patrick was born in Detroit, MI, and was the son of the late Max and Elizabeth Smukal Forma.

He is survived by his daughter, Cassandra Forma; life partner, Judy Ostheimer; sister, Maxine Sebes (Al); brother, Larry Forma (Sherry); sons of the heart, Paul Cape, and Kevin Ostheimer (Kristen); and daughter of the heart, Heidi Salciccioli (Anthony).

He was preceded in death by his sister, Paula Haddow.
