Dr. Amy Conley-Sallaz, age 53, of Morehead, Kentucky, passed away Saturday, August 17, 2024, at St. Joseph Hospital in Mt. Sterling, Kentucky.

Born September 24, 1970, in Boyd County, Kentucky, she was the daughter of the late Carl Conley and Ruth Anna Bradford Conley. In addition to her parents, she was preceded in death by one sister, Carletta Conley Murnane, and a brother-in-law, Danny Ray Jackson.

She is survived by her husband, Robert Brian Sallaz; four children, Carly Ann Sallaz, Emily Ruth Sallaz of Morehead, Kentucky, Rebecca Sallaz of Winchester, Kentucky, and Cody Sallaz (Brielle) of Morehead; two special grandchildren; one sister, Barbara Ann Jackson of Carter County, Kentucky; a special niece, Blake Murnane Kemper (Adam) of Ft. Thomas, Kentucky; extended family and friends.

Amy completed her undergraduate studies at Morehead State University.  She earned her medical degree at the University of Louisville School of Medicine; completed her residency in Family Medicine through the University of Kentucky's Rural Training Track program in Hazard; and a residency in obstetrics and gynecology at Marshall University, Huntington, West Virginia. She was presently working at UK KDMC Family Care Clinic in Catlettsburg and was a former physician at St. Claire Regional Medical Center for 22 years.

Dr. Amy Conley-Sallaz was the recipient of many honors during her career including the First Annual Family Medicine Medical Education Volunteer Faculty of the Year Award; the 2010 Outstanding Community-Based Faculty Award presented by the Kentucky Primary Care Association; 2009 Family Medicine Preceptor of the Year Award presented by the Eastern Kentucky Area Health Education Center (AHEC); 2008 Family Medicine Resident Faculty Teaching Award; and the 2007 AAFP Pfizer Teacher Development Award. She was also a Cultural Medicine Fellow through White Memorial Medical Center.

In spite of her many accomplishments in her career, Amy viewed her daughters as her greatest success. 

A celebration of life will be held 11 a.m. Thursday, August 22, 2024, at Northcutt & Son Home for Funerals Memorial Chapel with Brother Aryn Merritt officiating.  Burial will follow in Forest Lawn Memorial Gardens.

Pallbearers:  Terry Kidd, Chris Thomas, Cody Sallaz, Tim Hamm, Alex Swim, and Felipe Mazaba.

Visitation for Dr. Amy Jo Conley-Sallaz will be from 6 p.m. to 8 p.m. Wednesday, August 21, 2024, at Northcutt & Son Home for Funerals, 400 Fraley Drive, Morehead, KY 40351.

In lieu of flowers, the family requests memorial donations to Hope Pregnancy Center, 167 E. Main Street, Morehead, KY 40351.
