John Richard Madison was born April 3, 1951 in Toledo, Ohio. John passed away on January 12, 2021 at Hospice of Northwest Ohio.

John is preceded in death by his parents, Lee G. Madison and Ruby Madison "Big Mama"; brother, Willie Madison and George Madison; sisters, Dolores Jackson and Frances Barnett.

He leaves to mourn his passing, life partner, Cynthia Jackson; children, Delbert L. Jackson, Richard L. Jackson, DeWayne L. Jackson, and Tasha; brothers, Lee Madison, Edward Madison, Frederick Madison, and Cornelius Madison; sisters, Sheila Madison and Juanita Madison, along with other close family and friends.
