John F. Polak, age 83 of Effort passed away with his loving family at his side on Wednesday, November 24, 2021 at home. John was the loving husband of Mary Ann (Walsko) Polak. They celebrated their 59th Wedding Anniversary on May 19th. He was born in Prague, Czechoslovakia on May 1, 1938, son of the late John and Mary (Jusko) Polak. John had worked as a conductor for the New York City Transit Authority in NYC For 28 years until retiring. He served our country with dedication and courage in the U.S. Navy. John was a member of Our Lady Queen of Peace Catholic Church in Gilbert. He was a member of the Knights of Columbus Msgr. Connell A. McHugh Council 7141. We have been blessed with the presence of John in our lives. He will be deeply missed by his loving wife: Mary Ann of Effort, his sons: John Polak of Effort and Richard Polak and his wife Paula of Hawthorne, NY. He was the loving grandfather to his five grandchildren: Michael Polak and fiancé Allyson, Joseph Polak and wife Christine, Mark Polak, Kristina Polak and Dianna Polak and fiancé Nicholas. A Mass of Christian Burial will be held 11:00 AM, Monday, November 29th at Our Lady Queen of Peace Catholic Church in Gilbert.
