Thomas J. Maddex, 64 , of Prospect Heights, formerly of Morton Grove and Rogers Park, Chicago. Thomas passed away September 10, 2021 at his home. He was born March 19, 1957 in Evanston, IL. Beloved husband of Sharon Maddex, nee Roy for 40 years. Loving father of Thomas (Sheri) Maddex. Cherished grandfather of Alexander, Elijah, and Genevieve. Fond uncle of many. Preceded in death by his siblings Michael Maddex, Mary Pat Maddex, and Barbara Maddex Rankin. In lieu of flowers memorials may be made in Thomas's name to the Pulmonary Fibrosis Foundation, 230 E. Ohio St., Chicago, IL 60611. Memorial visitation Saturday, September 18th, 9:00 A.M. until Funeral Mass 10:00 A.M. at St. Raymond de Penafort Church, Elmhurst Rd.
