<p>Our wonderful husband, father, father-in-law, brother and grandpa, Chuck Mayo, passed away on May 20, 2024, in West Linn following a long medical journey. He had a life filled with love, family, friends, career satisfaction, fun, travel, sports, and kindness. We miss him immensely, but to experience profound grief one has to have experienced profound love. Chuck loved us all so much and we loved him.</p><p><br></p><p>Chuck was born at Emanuel Hospital to Lloyd and Alice Mayo on January 28, 1950. He graduated from Jefferson High School and attended Portland State University. In 1969 he married his high school sweetheart, Kathy Fowler, and after basic training for the Army Reserve he transferred to Oregon State to attend pharmacy school. Following graduation Chuck participated in a pharmacy residency at the Medical University of South Carolina. He completed his Master’s degree in pharmacy at OSU in 1976.</p><p><br></p><p>His pharmacy career included positions with Sacred Heart in Eugene, Mercy Medical Center in Roseburg, Legacy Good Samaritan, OHSU, Deaconess Hospital in Evansville, Indiana and finally 25 years with Good Samaritan Hospital in Corvallis. During his career he mentored many young pharmacists, implemented new pharmacy procedures for open heart surgeries, kidney transplants and IV advancements.</p><p><br></p><p>In addition to his career, Chuck was grateful for his family, his wife Kathy and son, Brian, his amazing daughter in law, Heidi and wonderful granddaughter, Maci. They were the greatest joys of his life.</p><p><br></p><p>Chuck spent many years coaching Brian’s sport teams, playing city league sports, running football and March Madness pools, and attending games at OSU. He loved golf, the Beavers, going to the gym and hanging out with his friends.</p><p><br></p><p>He is survived by his family, including sister Karen Mayo of Corvallis.</p><p><br></p><p>There will be a Celebration of Life on July 22, 2024, at Trysting Tree Golf Course in Corvallis at 11 am with a round of golf on Chuck following the celebration and reception.</p><p>Memories can be posted, and information found at In lieu of flowers contributions in Chuck’s name can be made to the OSU College of Pharmacy or the Pharmacy Foundation of Oregon.</p><p><br></p><p>Please RSVP on the evite link ( ) so we can get an accurate count for food, tables, chairs, programs, etc. The Celebration of Life and Luncheon will be held from 11 am - 1:00 pm.
